Holiday Wreath Order Form

There are several options for ordering your FMA holiday wreath. You may print out this order form and return it with your check to a music student or to a member of the FMA.

Or you may fill out the form below and submit it to place your order for a holiday wreath. After you fill out the form, your order will be sent to the FMA. You may then choose to pay online using PayPal or you can pay when you pick up your wreath at the high school on your selected date. You can also pay at any FMA meeting or by sending a check to FMA, 120 South Street, Foxboro, MA, 02035.

The wreaths are $35 each. Checks may be made payable to the Foxboro Music Association. All proceeds go to the FMA for the enrichment of activites in the Foxboro Public Schools.

The deadline for orders is 11/19/2009.

Be sure to mark your calendar with your pick up date.
